Share what you want to make, how it will be used, the approximate dimensions, and the quantity. If you already have a model, you can attach it to the form.

For an app question
Include the app name, your iPhone or iPad model, software version, and what happened. Screenshots are helpful, but please remove any personal information first.
For an existing request
Include the reference shown when you sent your request so we can connect your message to the original details. Please do not include payment-card numbers or passwords.
